About Diagnostic Error
In an ideal world, diagnosis would be accurate, timely, efficient, and above all, it would be safe. We don’t live in that world. In our world, diagnosis is missed, wrong, or seriously delayed in some 10% of cases. These are diagnostic errors, and many of these will be harmful.
